Annual Ministerial Report

To meet the membership requirements of the Evangelical Church Alliance, you must submit a detailed yearly report utilizing an Annual Ministerial Report Form. This report will comprise ministerial activities exclusively from January 1 through December 31 of any particular year, and must be submitted by April 1 of the following year.  Those who have not submitted this report by that deadline will by policy not be in good standing with the ECA.

Before submitting a current year's annual report, please be certain that you have submitted all previous years' reports since you became a member.  The 2014 Annual Ministerial Report and annual ministerial reports for successive years are web-based and to be completed and submitted though the ECA website. 


How to submit previous years' reports

To submit a report online from an earlier year, you must have Adobe Reader installed on your computer.  If you are currently using a Mac computer you will need to either fill out a hard copy of this form and mail or fax it to our office, or you can go to a PC and use the interactive version we have available on our website.  Mac is not compatible with the Adobe Reader.  We apologize for the inconvenience.   If you have any questions, please contact our office at (815) 937-0720. You may download Adobe Reader here.  You may also open the report, and after printing it, complete the form by hand. This form must not be altered in any way. It must also be filled out completely, and each response limited to the space allowed. Please do not insert any comments like, "See Attachment."
