ECA's Policy for Replacement of Misplaced or Lost Credentials
Upon receipt of an approved Candidate Evaluation Questionnaire, (ECA Form 127) and the first Annual Membership dues payment, a candidate who has applied for membership as a Licensed Minister with the ECA shall receive a Wallet-sized Credentials Card, a current Credentials Certificate, an ECA Ministers Manual, and a subscription of “The Evangel.”
Upon receipt of an approved Candidate Evaluation Questionnaire, (ECA Form 127) a completed Report of Ordination (ECA Form 144) and the first Annual Membership dues payment, a candidate who has applied for membership as an Ordained Minister with the ECA shall receive a Wallet-sized Credentials Card, a current Credentials Certificate, an Ordination Certificate, an ECA Ministers Manual, and a subscription of “The Evangel”
Additional or Replacement forms are available upon request according to the following criteria.
Lost Credentials Certificates, Credential Cards will be replaced upon the completion of a Credentials Replacement Order Form, explaining the nature of the loss of the original documents, provided that the person is a member-in-good-standing with the ECA.
The replacement certificate will be marked “Copy”.
